When you visit one of the application sites to have your PTL application processed; as per the joining checklist you will have to bring proof of the benefit you are claiming/or which you are named on together with a separate recent proof of your address such as a recent Council Tax or utility bill.

Proof of Address

All applicants: Official proof of address required for all applicants e.g. recent utility bill or Council tax bill. For child applicants proof of address could be child benefit award letter or any other benefit claim which lists the children showing the home address. JSA (income based) can only be proved by showing all of the three-page benefit statement letter that is issued when first in receipt of benefit.

Or the PTL office can provide you with a PTL confirmation form which you can use to get the Job Centre to confirm and stamp.

Pension Credit Guarantee The award letter or a recent letter confirming receipt of benefit
Working Tax Credit & Free NHS Treatment (Dental, optician as well as prescriptions). The award letter and NHS Tax Credit Exemption Certificate showing customer is in receipt of benefit and free NHS Treatment (Dental, optician as well as prescriptions) (credit card size and HC2 Certificate.)
Child Tax Credit & Free NHS Treatment (Dental, optician as well as prescriptions) The award letter and the NHS Tax Credit Exemption Certificate showing customer is in receipt of free NHS Treatment (Dental, optician as well as prescriptions)
NHS Low Income Scheme. The HC2 Certificate for NHS Treatment (Dental, optician as well as prescriptions)
Partner of someone on benefit. Proof that you are included in the benefit claim e.g. award letter for your partner naming you as being included
Youth Training / Employment Training Letter from employer confirming that they are on a Youth Training or Employment Training Scheme.
Full time students (year 12 or older) An official letter / Student Certificate from 6th form school, college or University on headed paper confirming full time education status.
Employment & Support Allowance The award letter or a recent letter confirming receipt of benefit
