You can use a number of documents as proof of your eligibility for Passport to Leisure.
Proof of Address
All applicants: Official proof of address required for all applicants e.g. recent utility bill or Council tax bill. For child applicants proof of address could be child benefit award letter or any other benefit claim which lists the children showing the home address. If you have any difficulties please call the PTL office for advice on 08000 43 43 43.
Housing Benefit
Recent letter from relevant Housing Benefit department confirming receipt of benefit.
Council Tax Benefit
Recent letter from relevant Council Tax department confirming receipt of benefit.
Income Support
The award letter, or a recent letter confirming receipt of benefit.
Job Seekers Allowance (Income Based)
JSA (income based) can only be proved by showing all of the three-page benefit statement letter that is issued when first in receipt of benefit.
Employment Support Allowance (Income Based)
The award letter clearly stating the ESA income based is currently being paid.
Guaranteed Pension Credit
The award letter or a recent letter confirming receipt of benefit.
Working Tax Credit & NHS Tax Credit exemption certificate
The award letter and the NHS Tax Credit Exemption Certificate showing you are also in receipt of free optical and dental care.
Child Tax Credit & NHS Tax Credit exemption certificate
The award letter and the NHS Tax Credit Exemption Certificate showing you are also in receipt of free optical and dental care.
Universal Credit
The Universal Credit statement/journal confirming your earnings level.
NHS Low Income Scheme
The HC2 certificate for free NHS free optical and dental care.
16-19 in FTE/Training named on parents benefit (Pay as you Go)
Proof of non advance FT student status/inclusion on approved training scheme (see below) and proof you are included in parents/guardians benefit claim:
Enrolled in non-advanced education:
GCSE’s and A Levels
NVQ level 1,2 or 3
BTEC national Diploma, national Certificate and 1st Diploma
SCE higher grade or equivalent.
Enrolled in an approved training scheme:
Skillbuild
Foundation Modern Apprenticeships or Modern Apprenticeships
Supported Young Adult
If you fall into this category you will have to get your Social Services representative to approach the PTL office who can organise proof of eligibility and entry onto the scheme.
Asylum Seeker
A NASS Home Office identity card (ARC) as proof, or hold a NHS HC2 or have an authorising letter from the Welsh Refugee Council.

PTL Student
A current letter from school, college or university that shows, the start and end dates of the course, duration of the course and weekly number of hours of the course.
